Stack up with spacers 5mm at a time, start from the bottom & work up. I set it so that it pulls up easily but doesn't make the back wheel stuff too hard, if it's too high I find it's actually harder to pull up, there's an optimum hand/foot distance for any bike or rider, find it.

Luckily my perfect spot is right at the end of the steerer tube.

I've got big Da Bomb Tactic risers, but if I put Trialtech hi rise bars on then it's not quite high enough, there's a fine line.
